I seem to remember this happening once when I first tried syncing my Clip+ with an external card in MTP mode. If I remember correctly, I formatted the external card and switched to MSC mode. I haven’t had any problems since.
I believe you can “overfill” these cards by having so much music on them that there is no room for its database. I had to adjust the amount of songs that MediaMonkey puts on my Clip+ to avoid this problem.
