Extreme Pro 64 GB Reading/Format Issue

Ok, so I have a Sandisk Extreme PRO SDXC UHS - I 64 GB card from few weeks. I have no problem until yesterday. Card was full (one very big video file) as always soo I copy the file and press format in exFat on WIn 7 and the power on my card reader switch off. It’s not card related just my USB hub switch off (it;s not important why, it was normal ).

Then whole formatting process freeze. I can’t do anything so I take card from reader to renew formatting.

After inserting it back again windows don’t read this card. Nothing show up on My Computer just like card reader was empty. I switch computer to another operating system OS X (it Mac Pro witch Win 7 on it) and try to do it with disc manager.

In OS X card show up for a moment then disappear and over again. I can’t format it in 2 sec…

What program can be use to read it and format it as it should be ? I’m broke my card or what ?

Yeah I manage to format it once with SD Formatter 4.0 but it format it with 132 kb allocation and my camera recommend 128 (default in win 7 ) so i try to format it again with WIn 7 and formatting (quick option) goes for 3-4 min then the error that win couldn’t format this card is appearing. When i try to format it again with SD Formatter it freeze. Same with NTFS…

The only good option is that at least now Windows is reading this card. I mean i can see card and size in My Computer and open it.
