I use JVC Air Cushions (HA-FX66R) and have been very pleased with them.  Comfortable, great noice isolation, and sound amazing (well, at leat to me!).

The Sennheiser MX580 at around $25 sounds quite good. For the price, the $6 Panasonic RP-HJE120 IEM sounds quite good.
