cruzer blade 4gb doesn't formet

i have a cruzer blade 4gb  Model-SDCZ50-004G

when i enter my usb a pop up come to formet it.but when i try to formet this it shows it write protected.

when i see it’s propertis it show nothing or 00MB.

so what i do now

I checked the KB as pointed out. Can we assume already that this is a hardware failure?
