If the issue occurs anytime the drive is connected the issue may be a related to the SSD and the SATA chipset on your motherboard. Can you let us know what type of motherboard is in your PC?
this could be a bad drive as well. did it ever work or has this been hapening since you purchased the drive? if you have a USB harddrive enclosure can you attach the SSD using the USB enclosure? when connected as an external drive do you see similar issues?
